We help seniors get out there and take on the world.
Cardiac health, diabetes management, joint care, and wellness exams. Giving you more life to live.
Cardiac Health
Senior adults aged 65 and up are more likely to suffer from cardiovascular disease, which is the catch-all term for problems with the heart, blood vessels, or both. The wear and tear aging has brought can cause changes in the heart and blood vessels, increasing a person’s risk for certain medical conditions.
We help manage a variety of the more common cardiovascular conditions here at Sunnyvale Primary Care. While treatments will vary, they can range from simpler things like lifestyle changes and prescribed medications, to surgery that involves stents, pacemakers, or ablation.
Diabetes Management
Diabetes is a disease where the blood glucose (blood sugar) levels become too high for your body to process normally. As we age, our dispositions to the incidence of diabetes increases as our insulin reserve can start to run low and blood sugars begin to elevate unchecked. There is also a large genetic component to the development of diabetes. Since elderly people have been exposed to sugar longer than others so their chances of developing higher blood glucose levels is greater.
We can help manage your diabetes through a personalized plan that will likely include an exercise regimen and strict dietary guidelines. We will also need to monitor your blood glucose levels and adjust the plan as needed as we progress.
Joint and Bone Care
As people get older, either through natural wear and tear or injury, cartilage and muscle mass diminish. Add to this any excessive weight gain and hormonal changes and you’ve got a recipe for potential joint and bone conditions.
Once this bone is lost, it cannot be regained and steps should be taken to prevent further loss going forward. Our joint and bone care process will be customized to your specific needs, but you will likely be asked to eat a more nutritious diet, start an exercise program, and look into beneficial supplements as we keep an eye on your bone mass and treat any existing diseases such as osteoarthritis and rheumatoid arthritis.
Wellness Exams
Wellness exams, or wellness checks, are a simple series of screenings to check the overall health of a person. As people age, the particular types of exams done may differ. For Senior Wellness Exams, we typically suggest:
- Height, weight, and blood pressure measurements
- Vision and hearing tests
- Colon cancer screening
- Hepatitis C test (individuals born between 1945 and 1965)
- Abdominal aortic aneurysm screening
- Bone density scan (women over the age of 65)
- PAP test and mammogram (women)
This is not an exhaustive list, and the particular tests our doctors may administer depend on several factors, one of which being your medical history.
